Submit the work-injury report within eight days
The report is generally submitted by the employer, self-employed person or activity organiser to the HZZO regional office for the person's residence or temporary residence, or the employer's registered office.
Start the procedure if no report was submitted
At your request, the report is submitted by the selected family-medicine doctor; the insured person may also apply to HZZO, and after death a family member may do so.
Provide the requested documentation
Attach medical findings and evidence connecting the injury with work.
Follow the decision and related rights
Recognition affects salary compensation and other insurance rights.
