Determine the basis for insurance
Check whether an employer or another obligated person registers you, or whether you must apply yourself.
Check the eight-day deadline
Registration, change or deregistration must be submitted within eight days of the relevant circumstances arising, changing or ending.
Choose the appropriate form

Submit the registration to HZZO
Submit the registration and attachments at any HZZO regional office or branch.
