    Check the conditions before travelling

    You must generally be referred to the nearest contracted provider, have no entitlement to ambulance transport, and travel at least 50 km. Exceptions to the distance rule apply to certain groups, including children, people receiving haemodialysis and people with a recognised work injury.

    Obtain a travel order

    It is issued by your selected primary-care or dental-medicine doctor; when HZZO calls you to exercise a right, it is issued by an authorised HZZO employee.

  3. 3

    Have the travel order certified by the provider

    The contracted provider must confirm the completed examination, test or procedure. For repeated journeys, a travel order is valid with only one provider.

  4. 4

    Submit the completed travel order to HZZO

    A correctly completed and used travel order is also the reimbursement claim.

Important notes

  • Not every journey to an appointment automatically entitles you to reimbursement.
  • Choosing a different provider instead of the one you were referred to can exclude travel reimbursement.
  • The right to an escort must be determined by the selected doctor; children under 18 and certain fully incapacitated people are presumed to need an escort.

Common mistakes

  • The provider does not confirm the completed service on the travel order.
  • Confirmations from several different providers are collected for a repeated-journey travel order.
